Erectile dysfunction. There is a cause, and there are options.
Erectile dysfunction — known locally as mati pucuk — means difficulty getting or keeping an erection firm enough for sex. It is very common after 40 and is often linked to blood vessel health, blood sugar, hormones or stress.
Common signs
- Difficulty getting an erection despite arousal — often reduced blood flow; check sugar, blood pressure and cholesterol
- Erection that cannot be maintained — blood vessels or performance anxiety
- Fewer morning erections — more likely a physical cause
- Low desire, tiredness and a growing waistline as well — possibly low testosterone
Often a sign of another condition
The blood vessels of the penis are smaller than those of the heart, so erection problems can appear years before heart disease, diabetes or high blood pressure is diagnosed. Other causes include low testosterone, obesity, smoking, sleep apnoea, side effects of some medicines (for example certain blood pressure tablets and antidepressants) and emotional factors. Most men have more than one cause.
At Klinik Etika Kota Bharu
- Doctor's assessment — a confidential health and sexual history, blood pressure, weight and examination if appropriate
- Tests based on risk — blood sugar, cholesterol and testosterone when relevant, to find the cause rather than simply hand out medicine
- Treating the cause and lifestyle — controlling sugar, blood pressure and weight, stopping smoking and sleeping well all affect erections
- Prescription treatment and other options — prescription medicine only after assessment and only if safe for you; counselling, pelvic floor exercises or a vacuum device for selected men

What to expect, and cost
The first visit usually covers a discussion, examination and basic tests if needed. Test costs are explained before they are done. A follow-up after 2–6 weeks reviews how treatment is working. Your partner is welcome to join if you are comfortable.
A word on “ubat kuat”
Many men buy “ubat kuat” or so-called power pills online. Unregistered products can contain hidden drug ingredients that are dangerous — especially for heart patients taking nitrate medicines. If you use any product, check its MAL registration number on the NPRA website. A doctor's assessment keeps treatment safe and makes sure a cause such as undiagnosed diabetes is not missed.

Frequently asked questions
Can erectile dysfunction be treated?
For many men, yes. Erectile dysfunction often improves when the cause is treated — for example better blood sugar control, weight loss, a change of medicine or treating low testosterone. Prescription medicine also helps many men. Results vary, which is why assessment matters.
At what age is erectile dysfunction common?
It can happen at any age but becomes more common after 40. In younger men anxiety, stress, smoking and lifestyle are more common, but physical causes should still be checked.
Can I buy erectile dysfunction medicine at a pharmacy?
Medicines for erectile dysfunction are prescription medicines that need a doctor's assessment because they can interact with heart medicines and are not suitable for everyone. Avoid products without a valid MAL number on NPRA.
Is erectile dysfunction linked to diabetes?
Yes. Diabetes damages small blood vessels and nerves. Men with diabetes have erectile problems more often, and sometimes an erection problem is the first sign before diabetes is found.
How much does erectile dysfunction treatment cost?
It depends on the assessment, the tests needed and the treatment chosen. The doctor explains the costs before any test or treatment is started.
